Diagnosis
Ben is a tough case to crack. I believe Ben’s diagnosis is Bipolar II (269.89). He has not had any “full blown” manic episodes that we know of at this point which rules out Bipolar I. This is ruled out because at least one manic episode in lifetime is required for a Bipolar I diagnosis (American Psychological Association, 2013). Ben’s wife reports that Ben has become “excitable, extremely irritable, and has lots of energy” at times. This describes a hypomanic episode.
Rational
Ben has been having difficulty sleeping. He has been waking up early. He has had trouble with motivation to go to work and says he feels worthless. This points to Ben having a major depressive episode (American Psychological Association, 2013).
