Jennifer Broome, MA called me and requested that I see Dr. Nuthi’s patient concerning her high PHQ-9 score of ____ and lack of follow up with a behavioral health therapist or psychiatrist. After Dr. Nuthi concluded his visit with the patient and introduced me, he left the exam room with just the patient and I present. She explained that she has been diagnosis with Bipolar for over 10 years and in the past two years has been non-adherent to her behavioral health care due to unemployment and lack of income. I stressed the importance of compliance with medications, and treatment. She stated that she currently has Blue Cross Blue Shield and I identified four providers which she can contact to verify that they are covered by her insurance. She stated that she will follow up with them. Although the patient stated that she has thoughts of suicide, she denied being suicidal while we were talking. I asked her to call 911 if she does begin to feel like harming herself. …show more content…
Additionally, she made a request to learn more about what Bipolar is. I exclaimed that I will call her this afternoon with information on food pantries and that I would send her information on bipolar education. I suggested that in addition to finding a therapist and psychiatrist that the patient also keep a “Gratitude Journal” and try exercising to help with her depression.
I downloaded from TherapistAid.com a handout on “What is Bipolar Disorder”, “The Causes of Bipolar Disorder” and a “Gratitude Journal”. I called the patient and provider her with the number to Foodline and with the number for NAMI – Huntsville, AL support group which meets the 1st Tuesday of the month in the United Way building.
