The patient is a 46 year old male who presented to the ED under IVC for non-compliance with medications, alleged auditory and visual hallucination, and not bathing regularly. The patient denies suicidal ideation, homicidal ideations, and symptoms of psychosis. Patient denies depressive symptoms. Patient reports his mother call the police today because their neighbor saw him at her front door.

During the time of the assessment the patient was calm, 4X oriented, malodorous, and sloppy. The patient does not appear to be responding to any internal stimuli. The patient reports he does have a history of Bipolar, which he was diagnosed 10 to 15 years ago, however has not taken off the medications 3 years ago. Nothing in patient records show
