Reason for Visit: Bilateral Knee Contusion
S: TM was walking in cafeteria when she slipped on a wet floor. There was a Wet Floor sign in place but she wasn’t pay attention to it because she was busy greeting other TMs. TM denies any issues, rates her pain at 0.5/10, some discomfort.
O: Bilateral Knee: some rudeness noted on the knees, no edema, no effusion of the knees, denies tenderness with palpation. full active and passive ROM; Knee joint stable, without locking, and catching.
A: Bilateral Knee Contusion
P: ICE X 20 minutes now, Ice at home
Disposition: Full Duty, Follow up Prior to next shift
