On 5/24/2016, CM met with the client to complete Bi-Weekly ILP Review. In the meeting client was dressed with proper attire for the weather. She was cooperative, friendly and forthcoming with information.

SOCIAL SUPPORT UPDATE: Client reported she haven’t heard from her daughter she left numerous messages but no avail. She continues to mention her daughter is busy working. Client report no issues or concern in the “J” dorm.

EMPLOYMENT UPDATE: Client continues to be unemployed, and WECARE/FEDCAP -16/work limited. Client continues to report she hasn't received any BTW/FEDCAP letters. Client continues to mention she is seeking Factory jobs with a Chocolate Factory located in Channel Beach, Far Rockaway and she was told to come back in the summer.

HOUSING UPDATE: GP housing package submitted and circulating. Client has an unrealistic housing plan.
Client continues to report her plan of action to move out the shelter. Is to secure employment with a seasonal Factory then apply for Unemployment. Client reported she will be actively looking for employment in the summer and once she secure employment she will be moving out on her own.

REVIEW ILP AND CLIENT RIGHTS AND CLIENT CODE OF CONDUCT:
Client must adhere to the shelter rule and regulation and to 10pm curfew.

WORKER ASSESSMENT: Client continues to show the ability to plan for herself and she will benefit from the services being offered at SRO or she can live independently.

Plan of action: Client must keep all medical appointments and provide documentation as requested, client most accept first suitable housing offers, Client must comply with and maintain eligibility for PA. Client must notify staff of any changes.
Client must notify staff of any changes and provide proof (i.e., budget letter) upon request. CM reviewed the Bi-weekly ILP. Client agreed and signed ILP. Next schedule appointment
