On 11/1/16 at approximately 1700 Medical was requested to respond to Intake Holding to assess offender Bell OID 249710 for reporting to staff that she was feeling like she was going to have a seizure. Upon arrival at intake offender was sitting on a bunk in no acute ditress. Offender stated that she was scared to go to seg because she didnt do anything and that was making her to feel like she was going to have a seizure. During assessment offender was crying and saying that she argued with another offender and she didnt want to get introuble because she has been good everyday. Offender was assessed and her vital signs were withing normal limit. As soon as I was done doing assessment offender started shaking, kicking her legs, hypeventilating,
