On Wednesday March 09, 2016 5:30pm at 11 Medical Park on the RS1 Patient Donnell Bennett attacked another patient Charlotte McCoy. Ms. McCoy was sitting on the couch in the day room on RS1. Mr Singer is a 61 year old male who presented to the ED with uncontrollable behavior at his living facility. He does not presents to ED with aggressive behavior or agitation. On November 23, 2016, I was informed by nurse Rockhead that a patient in the waiting room threatened to shot the clerk with a gun. The patient was also reported threatened to do harm if caught outside of work. Although I was made aware of the incident until after it had occurred, I followed up with all members involved and security. I also informed my director Mindy Cody of the incident. Reporting party (RP) Residents were not being awakened for meals if the residents are asleep during meals the caregivers don't wake them. There is no food provided for the residents once they wake. Hospice residents who are sleep and don't make it into the dining hall they are not fed. Resident John Shock would talk about bodily functions making the other residents uncomfortable. Over a three-week period thirteen patients on the night shift suffered violent seizures that where followed by cardiac and respiratory arrest. Of the thirteen nine of the patients died and the nurses described the patients’ seizures to be similar to grand mal seizures that epileptics suffer. Seizures usually do not occur in cardiac patients. Soon after the cardiac care/ intensive care unit Community Hospital of the Valleys closed and the defendant took a job at San Gorgonio Pass Hospital. In Baker v Carr, the majority made up the basis for asserting jurisdiction because the Constitution, the supreme law of the land, did not give it the power to do so. FD responded to the listed location for a person with an altered mental status. FD arrived to find an alert, bedfast patient with complaint of AMS per family. Pt’s daughter advised that her mother is displaying abnormal signs of AMS, general weakness, and she also has a Hx of COPD, restrictive lung disease, chronic renal failure, pulmonary hypotension with five heart stents. As FD began initial assessment, EMS arrived on scene and care was transferred. FD assisted with loading the patient onto the stretcher and into the ambulance without incident.…

Close to the end of my shift, a nurse approached the pharmacy asking us to do a medication reconciliation on a patient who had recently been admitted to the floor. This had been his second time being admitted to the hospital in the last week so he was no stranger to the floor.
