Baccalaureate-Level Clinical Nursing Practice: A Clinical Analysis

I chose this article because in my personal life, I have interacted with healthcare professionals that want to prescribe depression medication without listening to their patient. Not only do SSRIs have adverse effects in the older adult population, a more whole person assessment is not given to get to the root of the problem. SSRI are extremely beneficial, but addressing the real problem i.e.; social isolation, polypharmacy, or loss of autonomy, may prevent healthcare complications.
B. Direct Quotation from the Article Type or copy and paste the exact words.
“Elderly patients are more likely to develop hyponatremia with the use of selective serotonin reuptake inhibitors (SSRIs). Use of selective serotonin uptake inhibitors also has been shown to increase the risk of upper gastrointestinal bleeding” (Paauw & Wallace, 2015).

How I Could Apply the Quoted Material in Baccalaureate-level Clinical Nursing Practice Explain how you could use the information in the section that you quoted in your baccalaureate-level nursing practice.
I will integrate this knowledge into my practice while working with “depressed” older adults. Exploring and listening to the patient’s story will help me provide better patient teaching and possibly helping them with their depression enabling them to get off SSRIs. This direct quote taught me the importance of patient teaching about their medications and drug interactions. For example; watching for signs of hyponatremia and education on diet, and not using drugs that cause damage to the upper GI tract (NSAIDS). Understanding the changes that occur as we age, helps me understand specific pharmacokinetics and further develops my nursing

Advance Practice Registered Nurse (APRN) and a registered nurse (RN) are mandated to follow the regulations in their states of practice. In terms of regulations, an APRN has focused scope of practice which has limitation to practice, unlike the RNs with broad scope of practice (Hudspeth,2009). In the state of Texas, an APRN practices in areas beyond the scope of the RN, such as diagnosing and prescribing. For instance, once an APRN is educationally prepared as an acute care NP, the practice can only be acute and not primary care setting except further education is provided to practice in primary care setting, this believed, that the APAN does not have the didactic and clinical content to practice in a different area (Texas board of nurses…

The State of Texas currently has many restrictions on Advance Practice Registered Nurses (APRN). We are one of thirty-two states in the United States that require Nurse Practitioners to work in a collaborative/supervisory relationship with a physician. Connecticut recently became the 18th state to allow Nurse Practitioners to practice independently (Lowes, 2014). Bill SB 751 for the 84th legislature was introduced by Senator Jose Rodriguez. This bill is aimed at granting Nurse Practitioners in our state autonomy from physician supervision and full prescriptive authority.…

12.00 TREATMENT OF POST MI DEPRESSION When depression is suspected in an MI patient or the screening tool reveals depressive symptoms, the patient needs to be referred to a Psychiatrist for further management. Adequate and prompt management of depression can improve the quality of life in MI patients . Nurses use evidence informed practise which is the method of gathering evidence and incorporating clinical expertise, patient preference, expert opinion and other resources (Canadian Nurses Association, 2010). In addition, nurses will use Carper's ways of knowing as describe above and will bring their practical knowledge brought in from clinical experience. This clinical experience is a valuable asset as they understand the environment and can incorporate it into their own experience of practise. Lastly, praxis is the "practice, as distinguished from theory; application or use, as of knowledge or skills" (Myers, 2009, p. 1497) During this project, I have been able to use my knowledge acquired throughout the nursing program and apply it…

This article about antidepressants starts by talking about how the use of antidepressants has increased throughout the decades. The article suggest that depression is being misdiagnosed in many patients who think have depression when they really do not. These people are turning to antidepressants to address and deal with feeling sad and stress in their life. The article suggests that depression can be properly diagnosed and treated with a psychiatrist, but people turn to their primary care doctors for a faster way to treat what they think is depression. Something I found interesting about this article was the fact that two-thirds of a sample of more than 5,00 patients who had been diagnosed with depression within the past 12 months did…

In the United States today, exposure to fast food ads, environment, socioeconomic status, genetic predisposition, overeating and unhealthy snacking has increased obesity in U.S. residents. According to the Centers of Disease Control and Prevention (CDC), more than one-third of U.S. adults are obese. Research suggests that obesity and not exercising is the leading cause of heart disease, which is the number one cause of death in America (CDC, 2016). As the number of obese patients enter the hospital, so does the need for more health care workers and the appropriate equipment. Nurses have to provide basic care to every single patient, which ultimately leads to back injuries in the nursing field.…

The crippling effects of depression lead many people to take drastic measures in order to find some type of relief. The most common way for people who suffer from depression is to seek out and find doctors who readily prescribe antidepressants. However, studies suggest that antidepressants are only moderately effective, especially during the first six to eight weeks of use. Some in the psychiatric field believe that antidepressants only have a moderate to low ability to fight off depression. Medication Management Antidepressants are the front line of defense in the treatment of bi-polar disorder, clinical depression and many anxiety disorders.…

During my research It was noted that an “increased efforts are needed to enhance clinical training and knowledge in psychopharmacology among trainees and practicing clinicians”. (Gardner, 2014) these medications are not being prescribed for the actual intended use, safety warnings by pharmaceutical manufactures known as the black box labelling are put in place to notified the patients of their deadly side effects (Toronto Star, 2014). The adverse effects heavily outweigh the temporary benefits of what the health care providers are seeking to achieve. ‘These antipsychotic medications are not approved by health Canada for treating seniors with dementia they are used to sedate clients in nursing homes who are sometimes aggressive, wonder’s off and are easily agitated.…

Because of the stigma associated with mental issues, people are afraid to be prescribed antidepressants because they do not want to be labeled as “crazy.” People do not want to be labeled in such a way that will follow them the rest of their lives because in doing so they must admit to themselves that they have a problem that is not physical and that they can’t solve themselves by “snapping” out of it. All of this can be quite trying for the patient and can even make some of the symptoms worse besides there are also many side effects associated with taking an antidepressant. These same issues adults must deal with are even harder for children and adolescents to deal with because they are so young and are already dealing with the symptoms…

Senior citizens are known to commonly be taken advantage of in many situations because they are older. It is sad to find studies that prove this fact true mentioned in The Huffington Post that one out of five seniors get taken advantage of. Anyone in their rightful mind would imagine a doctor to try and help elders and their current problems instead of false prescribing medication and causing more problems. Not only is it sad that one out of five elders are being taken of, the drugs prescribed to the elders is even worse.…

Antidepressants should be highly regulated when distributed to adolescents. The prescription of antidepressants is often a "quick fix" to a permanent or complicated situation, and is, in many cases, ineffective or harmful. Various instances of suicidal events have occurred due to hap-hazard methods used when prescribing antidepressants. However, when used properly and under adequate supervision, these drugs can be live-saving and have very positive impacts on a child or teen's life. When evaluating a patient for a depressive illness or symptoms, the physician obtains a thorough history of the patient, and does a physical and psychiatric evaluation.…
