Phoenix is a female baby born on March 18, 2015 and weighed 6 pounds. She was assessed on September 24, 2015 at 10:00 am. She resides in Tallahassee, Florida with her mom, dad, and brother. She was born at 38.4 weeks gestation age. She was diagnosed with mild jaundice at birth. She is now 6 months, 6 days old. She stays at home with her father five days a week. She does not receive weekly therapy. She sees a Physician every two months. She had the 4 months required shots and scheduled to get 6 months shots soon. She enjoys playing with her immediate family.
For the first skills assessed for the cognitive behaviors. Baby Phoenix showed familiar activity by imitating the movements involved. Touches toy or adult’s hand to restart an activity. Reaches for second objects purposefully. Works for desired, out of reach objects. Showed interest in

Holds head steady in support sitting. Bears some weight on legs. Protective extension was apparent of arms and legs downward. Bears weight on hands in prone. Extends head, back and hips when held in ventral suspension. Rolls supine to side. Holds head erect when leaning forward. She can roll head to both sides. Sits independently indefinitely may use hands. Raises hips pushing with feet in supine. Bears almost all weight on legs. Lifts head and assist when pulled to sitting. Rolls to prone. Body righting on body reaction. Demonstrates balance reactions in supine. Protective extension of arms to side and front. Lifts head in supine. Holds weight on one hand in prone. Gets to sitting without assistance. Crawls backward and backwards. The mother stated that she has been crawling for a few days. Demonstrates balance on hands and knees.
