I just got off the phone w/Jay from BCBS. According to him in order for their system to bypass the need for additional documentation for ultrasounds that exceed the max. # of 3 due to high risk Modifier KX should be added to the procedure code. He stated that this is certainly the rule for BCBS of MI. Jay also said that this rule may also apply to BCN & out-of-state BCBS policies but wasn't certain. Hopefully, this will help w/the issue of benefits maxed & pt.
