Justification of 314.01 - Attention-Deficit/Hyperactivity Disorder (ADHD) Combined Presentation – Mild
Hannah has persistently displayed inattention and hyperactivity-impulsivity, which has effected her daily functioning and development. Hannah has displayed at least six of the inattentive and hyperactive/impulsive symptoms for more than six months and they are negatively impacting her academically and socially. She is struggling in school and has trouble making friends (Criterion D). She has difficulty with organization and leaves drawings half-finished (Criterion A1d). She has trouble concentrating and focusing on things, which means she is often distracted (Criterion A1b and A1h). She has bad time management and is often late for school and other events (Criterion A1e). Hannah does not seem to be listening when she is spoken to and her teachers have to repeat instructions to her numerous times (Criterion A1c). Hannah has lost many school items (Criterion A1g). Hannah has trouble sitting still in her seat (Criterion A2a). She often gets up out of her seat during class and story time (Criterion A2b) and continuously talks to other children and makes jokes (Criterion A2f). This interrupts class time and story time (Criterion A2i).…
