Robin reports feeling very anxious and irritable much of the time and would like to gain a better understanding of her problems. Robin’s affective and emotional state appeared sad and crying. Robin denies any suicidal and homicidal ideation. The main themes of the session were coping with feelings of anxiety, depression and substance use. Robin reported that she is struggling with depression and anxiety for more than ten years now mostly without treatment. She reported that she was once taking Paxil for her depression and it helped her, but when she ran out she never got it refile. I have been crying a lot and sometimes I don’t go outside because I can’t stop crying so I isolate myself”. She reported having several panic attacks a week and using substance to cope. Robin reported she has the support of her children and boyfriend and she is ready for to undergo treatment. We discussed her treatment options. And she express that she would like to attend the day program for both mental health and drug and alcohol treatment. I contact a therapist at the day program who gave a tour of the facility and informed us about the program in great detail. Robin really like the facility and asked many question that the therapist Karen answered. At one point Robin started crying and stated “I’m just so happy she will get the help I need”. We asked about the program eligibility and Karen informed us that it is recommended that she visit the program first to see how she fits in with everyone else and to determine if it’s the right fit or program for her. Robin is schedule to visit the program on 7/21/2016 at 9 a.m. after the tour we returned to my office and wrap up the session. Therapist and client created treatment goals and review them. At the end of the session Robin states that she has drug court on 7/20/2016 and requested a letter stating that she sought
