In review of your performance over the last several weeks, it has cause great concern. I have identified several areas that negatively impact the nursing vehicle for the Assisted Living Community. I am reviewing with you areas that will be addressed in your overall performance. I expect these issues to be addressed and your performance in these areas to improve significantly. Failure to do so will result in further disciplinary actions up to and including termination.
* Med Errors resulting under your supervision, you did not have a staff member review and select the correct dosage for administration, which would have been discovered if the order was read and reviewed by you and the Med Tech.
*Insubordination- you were loud and boisterous
