Reporting party (RP) stated that resident Ivan Carrillo (DOB: 04/25/91) was admitted to the hospital on 11/11/17. During this time, the hospital and the facility administrator have been corresponding about the resident's care plan once discharged from the facility. On 11/17/17, the resident's physician issued the resident's discharge order and the facility refused to accept the resident back into the facility. RP stated that hospital staff has been corresponding with the facility administrator, Jollie Ramos, who stated that his Director, Jose Mendoza, told him to, "hold boundaries and not accept the resident back.
