The reporting party (RP) state on 7/8/16 at approximately 5:30PM a visit was made to the facility to assess resident Bessie Masich DOB: 12/16/17. The RP was notified by the Home Health Aide Abby who reported observing a bruise on the resident right breast. The RP disclosed the home health aide worked Monday, Wednesday, and Friday. Friday was the day the home health aide observed the bruise. The RP and the hospice agency nurse Carla Cordileone-Murphy examined the resident and observed several bruises to the right side of the resident's breast, chest, neck, shoulder, and under arm. According to the RP there were 3 prominet bruises on her right side. Consequently the RP contacted the resident's son Nick Masich (559) 439-5283, (559) 696-7231, (559)
