CLIENT: Ms. G is a 40 year old, woman diagnosed with Schizoaffective Disorder, bipolar type. Ms. P was referred to the Community Treatment Team (CTT) at Family Services of Western Pennsylvania (FSWP) by her Mercy Behavioral Health case manager after failing to make several medical and psychiatric appointments and as result was no longer medication compliant. Ms. G has a history of manic acting out as well as well as episodes of depression and auditory hallucinations that occur outside of her manic episodes. G was put on the CTT case load which offers an individualized set of comprehensive services that are delivered on a no less than 3 times a week basis to the client in the community. Ms. G will get daily medication delivery, scheduling of all medical appointments as well as monthly visits with the team psychiatrist, weekly visits from therapists, access to all therapy groups offered, and a 24-hour crisis line just for those on the CTT caseload which is answered by a CTT team member. Additionally, CTT offers transportation to all medical appointments, food shopping, to governmental assistance offices, and anywhere else she may need to go to ensure all around wellness.
THEORETICAL/CLINICAL METHOD: The FSWP CTT is based off the Assertive Community Treatment (ACT) model that provides multidisciplinary, flexible treatment and support to people with chronic and persistent mental illness who require a level of care below inpatient hospitalization but above the level offered by other outpatient services. ACT is based around the idea that people receive better care when their mental health care providers work together.
