Work samples: Reading: Wyatt does not willingly participate in reading activities especially when his has to read a text and writing a response about what he has read. Discipline reports: Wyatt exhibits behaviors which affect his learning opportunities and those of his classmates. The behaviors seem to be the result of frustration as he struggles with assignments and classroom expectations. Wyatt has exhibited the following behaviors for resulted in consequences, such as signing the “Choice log”, “Silent lunch”, or “Independent recess”: incomplete/missing, task advoidance, difficulty following oral directions, following classroom/school wide expectations “R-O-A-R”. {Being responsible, orderly, accepting, or respectful}
IEP progress: According to Wyatt’s progress monitoring reports, Wyatt remains 3 to 4 grade levels in both reading, writing and math skills. He continues to work on reading multisyllabic words inclusive of two closed syllables with various blends, digraphs and irregular sounds. He is encouraged to seek assistance whenever he does not understand information given. Wyatt continues to struggle with math due to challenges with computations. He continues to struggle with mult-idigit multiplication and division processes. This continues to create difficulties with word problems. Even if he is given a calculator, he automatically shuts down with word problems and does not want to attempt to problem
