With the transition from student to practicing registered nurse comes a change in scope and personal accountability (Mellor & Greenhill, 2014). Upon entry to practice, the graduate nurse is expected to meet the Nursing and Midwifery Board of Australia’s 10 National Competency Standards for the Registered Nurse which are encompassed by the four domains of professional practice; critical thinking and analysis; provision and coordination of care; and collaborative and therapeutic practice upon entry to the workforce (El Haddad, Moxham & Broadbent, 2013; NMBA, 2010)
The use of the clinical simulation provides students with the opportunity to apply theoretical knowledge to clinical situations and gain experience in a ‘safe’ learning environment

Early identification improves patient outcomes (ACSQHC, 2012). Governance arrangements are in place to ensure policies and protocols for all organisations have response systems in place (ACSQHC, 2012). Standard 9 of the National Safety and Quality Health Standards (NSQHS) recognises that there needs to be systems in place to ensure that if a patient is deteriorating that this is recognised promptly, but also that appropriate action is taken (Australian Commission on Safety and Quality in Health Care (ACSQHC, 2012). Able to prompt graduate nurse to provide ISOBAR handover; to review bedside and inpatient progress notes for any clues to deterioration (if not already completed); and also prompts graduate nurse to check patient’s observations/vital signs and BSL (if not completed). The Patient
Mr Smith is pale, sweaty, and fidgety. Displays a decreasing level of consciousness during the simulation. Over the past few days, appetite has declined and BSL’s are fluctuating. Staff Development nurse

Anticipated Actions of Participants
Patient friend notifies nursing staff of patients altered health state.
Participant 1 (graduate nurse) enters room and reviews patient, checking patient bedside notes and reviewing handover sheet. BGL is measured, and a full set of observations is taken. Patient BSL is 3.6mmol/L. Following protocol, graduate nurse administers 150ml of orange juice to patient. S/he then contacts the treating clinician to advise as per the provided escalation protocol and informs shift
