Discussion: Heather reported that Jasmine PCP prescribed Mrilax for constipation and it has been helpful. Heather reported that Jasmine is still having diffculties focusing at school. Heather indicated that Jasmine needs to see a GI doctor because he stomach is always distend, and has a history of Acid reflux.

Emotional Wellness: Heather reported that she is still defiant, disrespectul and refuses to follow direction. Jasmine is stil working with her therapist (Wendy) to address this issues.

Physical wellness: BHH-HWE made an attempt to link Jasmine to an after school program, but Heather indicated that she doesn't have a car because Lewis (father) has started working.

Plan: BHH-HWE will search for an after school program that provided transportation.
