He and his family were made aware that after his mechanical aortic valve replacement, he would need to take anticoagulant medication indefinitely . He will be referred to an anti coagulation clinic where an anticoagulant dose that is high enough to prevent thromboembolism but low enough to prevent bleeding problems will be determined (Marques, 2009). Mr Jones was advised to be careful when using anti coagulation medication because of the risks associated with their use . This means that he will bruise more easily, bleed more heavily from minor injury, or even bleed spontaneously.

Periprocedural major bleeding is a strong autonomous predictor of early and late major adverse cardiovascular events and mortality (Bertrand, Larose, & Rodes-Labau, 2009; Verneugt, Steinhubl, & Hamon, 2011; as cited in Bernat et al., 2014). In an investigation of more than three million PCIs from the U. I attended the daily Anticoagulation Clinic evaluating the patients with different cardiovascular disorders and monitoring their INR accordingly. I actively participated in interacting with patients, discussing cases with Dr. Shah regarding indications of treatment, follow up and updated recommendations on principles of Anticoagulation. During discussions, I also came to know about new anticoagulant drugs currently in use or which are still under research. I also learned dietary counseling of patients on anticoagulation therapy and how an effective counseling can improve the drug compliance.…

There are two classes of antithrombotic medication, anti-platelet and anticoagulant, both help to prevent the formation of thrombi, or clots, which if get caught in a brain blood vessel are what cause an ischemic stroke (“Antithrombotic Therapy”, 2016). More than 83% of ischemic stroke patients are proscribed an antiplatelet, with ASA(aspirin) being proscribed almost 57% of the time (Bushnell et al., 2010, p. 1458). Twenty three percent of patients are prescribed warfarin, which is anticoagulant (Bushnell et al., 2010, p. 1458). Mr. Q has arteriosclerosis which means that a clot could become an embolus and travel to his brain reality easily and cause another stroke, which is why proscribing an antithrombotic is important for him.…
