The first goal was to increase the client’s understanding with regards to her disease to prevent injury and this was partially unmet. The client disclosed some learning associated with understanding that when she ambulates, her disease affects her activity tolerance. I clarified the pathophysiology of this disease, which made it more evident to acknowledge that her disease induced this increasing dyspnea during activities and not her age. Also, she showed a significant increase in appetite by eating her meals slower and more spread out during the day, I saw a significant improvement in her breathing patterns during her meals, she didn’t cough as much or complain of pain, which justifies that she has gained some understanding behind the purpose …show more content…
In the client’s medication administration record, she has no medications ordered aimed to control her mixed dementia. If anti-dementia drugs were prescribed, it could possibly improve her ability to focus, her memory and reduce disorientation, which all can positively impact her learning capacity. The second goal that aimed to encourage the client to participate in treatments to promote her health was also partially unmet. The client proved to have unstable compliment patterns because she failed to take her medications everyday. I educated the patient on the topic of how to properly use the Atrovent inhaler and its purpose, but she failed to return proper demonstration the following day and began refusing the medication because she wanted to sleep, which justifies her unstable commitments in improving her health. I noticed that the following week, her antibiotic, Ceftin by mouth, was changed to Tazocin intravenous, which signifies that her failure to comply with medication treatments initiated the change to a method of administration that she can’t refuse, proving that she didn’t comply with the prior treatment
