C: Ms. Smalls (MHP) and Mrs. Baxter (DSS Worker) had telephone conference with Ms. Johnson for monthly contact.

A: MHP answered Ms. Johnson questions about Angel’s behaviors. Ms. Johnson explain not being able to reach Angel’s DSS worker to ask questions about Angel’s whereabouts. MHP called Mrs. Baxter for Ms. Johnson would receive an update. Mrs. Baxter explain to Ms. Johnson the recent changes with Angel’s placement.

P: MHP will discharge Angel from the SAFY programs.
