The Parental Advisory Board of the North Gulfport School district would like to thank the High Mark foundation for considering our grant proposal to fund the “Shape It Up,” after-school fitness program. This grant proposal will detail our program’s mission statement. Next, we will begin by examining our communities needs and problems faced that will be addressed by this program.
Mission Statement
The goal of the “Shape It Up,” after-school fitness program is to provide our middle school-aged youth the resources needed to get physically active, promote positive self-worth, and encourage a healthier lifestyle. The youth at North Gulfport Middle School will have an opportunity to participate in an after-school program that makes getting fit fun.

The fitness coach will initially personalize a fitness agenda for each student with the provided equipment in the gymnasium. The physical education staff will be hired to insure the agenda is being properly cared out and to monitor the child’s monthly progress. A registered nurse will attend the after-school fitness program once a month to collaborate with the physical education staff. Every first Friday in the month, the physical education staff will track each child’s success rate and turn a designated checklist form into the registered nurse. The registered nurse will be responsible for relaying the information indicated on the checklist to our consulting physician and fitness coach to determine if any changes need to be implemented. The after-school fitness program will be always held in the school gymnasium and will be open from 3:30pm-4:30pm. Each student that selected the “Shape It Up,” program will receive an acceptance letter and will remain after-school for one hour daily to participant. The program will following the North Gulfport Middle School’s calendar and will not open on days the school is closed. Parents or Designated pick-up individuals are responsible for picking up their children in the gymnasium promptly at 4:30pm each day. Each child that participates in the …show more content…
During school hours, the gymnasium is used for physical education and extra-curricular activities. So, for the after-school fitness program it will need to be converted into a fitness center. The converted gymnasium will consist of the following equipment to accommodate the “Shape It Up,” program: a dumbbell set, mini-foldable trampoline, exercise bike, elliptical, and treadmill. Conversion will consist of moving all school day physical education equipment into the main storage closet. All equipment utilized during the after-school fitness program, will be sanitized by the janitorial staff. The program director will be responsible for insuring the physical education staff stored the “Shape It Up,” equipment properly and converted the gym back to its original state for school
