Some of my strengths in this domain are establishing/completing priority assessments, evaluating/monitoring nursing actions, and completing thorough assessments on clients concerns. During a shift I worked with a client who had just had an abdominal surgery. Upon doing my head-to-toe, he expressed that he had numbness in his upper thigh. I assessed this further, asking him about when it started, when its numb, looked at his CSMW and circulation, as well as asked about pain and sensation. He explained that it began during the night and had persisted into the day. I encouraged the client to ambulated to improve circulation and get his muscles moving.
