Hello friends! Welcome to this informal educational presentation. I am Rashmi Shrestha. I am a Registered Nurse and have been working at the bedside nursing for more than 10 years. Currently, I am working at medical surgical telemetry units. The practice of nursing is transforming. Today, I would like to discuss about the changing role of nurses, concept of continuity of care and various patient care delivery models like accountable care organization (ACO), medical homes and nurse- managed health clinics.

It is designed based on patient’s needs. The purpose of medical home is to improve accessibility, upturn care coordination and boost total quality care while cutting expenses. (The Medical Home Model of Care, 2012). It is a model for getting primary care excellence. The medical home relies on interdisciplinary team to meet health care needs of patient. The interdisciplinary team involves doctors, nurses, nutritionists, pharmacist, social workers etc. Medical home provides holistic approach to the patient. The focus is to incorporate with all features of health care that offers management of chronic illness, to improve physical health, behavioral health, and door to community-based social service. Patients are able to access services for 24/7. The medical homes offer primary care that is patient centered, committed to quality and safety, comprehensive, coordinated and accessible. NMHCs are operated by APRNs, mainly by nurse practitioners. They diagnose illness, prescribe medications and refer to the specialist as necessary. The clinics are frequently connected with school, college, university, nursing department and independent nonprofit health agency. The main purpose of NMHCs is to deliver primary care, promotion of health, and disease prevention to vulnerable group of population. These clinics deliver care to rural area, city and suburban societies.
