On this date worker received a call from Ms. Dana Long, Amedisys Hospice, for the purpose of following up on Mrs. Bertha Guthrie. Ms. Long stated they was providing services following her rehab days. According to Ms. Long, they have two concerns, Ms. Guthrie not receiving her meds during the day when Ms. Phyllis Guthrie is at work and she was wet during home visit. To address these issues they have called a family conference and are recommending they switch Ms. Guthrie to the pain patch instead of pill and the family hire someone to come and check on Ms. Guthrie every 4 hours. Ms. Long stated she would call the department if they noticed any health or safety risks in the
