Case Manager: “Although there is no cure for Alzheimer’s, you can still live a functional life. You would need extra assistance. I know a perfect place that can help you through this”

When you first arrived, we could not figure out what caused your symptoms such as forgetting to take medication and not eating enough for your age. You also asked questions when you did not understand. You provided great feedback about our service; we always hope to get feedback in order to improve our services. You made the right steps by setting up doctor appointment frequently. That was a major goal for us. We wanted to see if something physiologically was causing your symptoms. Finding out that you have Alzheimer’s disease could not have been possible if you did not attend those appointments. Antonio: “Wait, so you will not be my case manager anymore?”
Case Manager: “I will miss being your case manager, but the caseworkers that work specifically with the elderly are more trained than I am to help you with your condition. They will make sure you receive the best care possible, just like I always tried to do. Just remember to be open to their ideas as well. We will still have our weekly calls and site visits if needed for updates until you are assigned to a new case manager. I wish you the best of luck for finding your new home”
