Alexis Carter: A Case Study

On 11/06/2015 client Alexis Carter came to meet with CM Best for an ILP meeting. HS De La Torre approach to client and asked Ms. Carter to stay a little longer to develop a Housing Plan to explore barriers that family has to move into permanent housing.

Ms. Carter informed HS that last permanent address was 1240 Herkimer St. Brooklyn NY from 2/15/2015 to 5/30/15. Family was evicted from this location; client did not provide the reason for eviction. Family is currently residing at 1466 Grand Concourse Unit 5G Bronx, NY. Family composition is Alexis Carter (female HoH 21y); Cyle Perry (co-partner 26y) and London Spruill (daughter 3y). Ms. Carter informed HS that anyone will move with the family and anybody is pregnant at the moment.

    An additional issue is the change of residneces and the relocation of Mckenzie. Multiple relocation's have occurred with lack of proper communication. Since October 18th 2013 Alisha has relocated multiple times going from Gagetown,Bay City,Saginaw and now Chesterfield completely uprooting Mckenzie from her home and without making myself Jesse Bazan aware of addresses in said locations.
