CM conducted a face to face meeting for Akissa (youth) at the family’s home. In attendance were Jasmine Alexander (CM), Akissa Johnson (youth), Eileen Myers (Legal Guardian- Grandmother) and Monique Cox (FSO partner). Akissa noted Night School and therapeutic services are going well. CM was informed Akissa attends Night School at Martin Luther King Jr School (P.S. #11). Ms. Myers stated the family was not able to reach the program coordinator at the Horizon Center regarding classes. The family will inquired about parenting classes at Akissa’s upcoming OBGYN appointment in May. Akissa reported is scheduled to begin prenatal care soon and Akissa is excitement about motherhood. The team discussed Akissa progress and talked about transition. The
