Naomi- Writer met with Naomi at my work place. Care manager and Naomi were able to review care plan that was done on 10/17/16 and also crisis plan that was done on 9/2/16. Care manager informed Naomi that her case will be transferred to another care manager (Mary McNamara) since I will be leaving the organization soon. Care manager was able to introduce Mary to Naomi.
