The patient is an 89-year-old female who had an unwitnessed fall at home, fell forward on her face. She was walking out the kitchen without the aid of a cane which she normally uses. Past medical history is positive for CVA with residual right lower extremity weakness, she has had frequent falls and she has a chroni arrythmias, she has a permanent pacemaker in place. The patient had a full evaluation. Lab work is is without comment. The patient is awake, alert and oriented and can fully recall the incident and a achieve wishes to go home. Physical therapy evaluates the patient and passes her so that she is safe for discharge. The case is discussed with Dr. Agnelli who is transferred her over to outpatient service and I concur with this
