Aftercare is the follow-up monitoring of the patient’s condition after the primary treatment has been completed. Some patients with a serious mental illness are detained in hospital, so that they can receive care. Patients who are detained are eligible for care and support after they leave hospital. This is known as aftercare. Both the CCG and the Council have a duty to provide aftercare to patients who have been detained, once they leave hospital.
