Advanced Procedural Skills

For my first independent study as part of the Doctor of Nursing Practice Critical Care Fellowship, I focused on achieving competency with advanced procedural skills. This is consistent with Adult-Gerontology Acute Care Nurse Practitioner competencies that require AG-ACNPs to perform "specific diagnostic strategies and technical skills to monitor and sustain physiological function and ensure patient safety," (AACN, 2016). Through a multitude of learning opportunities, I believe my technical skills preforming advanced procedures has vastly improved and capacity to manage critically ill patients grown. The technical skills I focused on during the semester were arterial line placement, central venous line insertion, and advanced airway management. While not only having a setting to improve teamwork between nurse practitioners and our surgical resident colleagues, the exposure of cadavers gave me a better understanding of gross anatomy. Now when I'm placing a central line or arterial line, or positioning an ultrasound probe, I can better appreciate the structures of the human body. One aspect of advanced procedural skills I did not expect this semester is my own fear in completing the task. Throughout my education and training, I have always been eager to be able to place central lines. But once the responsibility fell squarely on my shoulders, I began to doubt myself and be fearful of the skill. At one point, I starting back away from opportunities to insert lines in the surgical intensive care unit. Fortunately, I work with excellent providers, both nurse practitioners and surgical fellows, who supported me while I regained my

There is an increasing expectation that these nurses should be allowed and made available with the smallest skills to work in a hospital, but they should also be equipped with the need to quickly learn and develop new and fresh knowledge related to critical care practice. A pre-graduation critical care study course has been provided as an immediate answer (Ferrell, & Virani, 2007).…

There I was, a young boy, waiting with my family in the hospital and the Surgical Assistant was taking care of my younger brother’s infected pre-auricular sinus. The scope of practice for APRNs consists of “parameters within which advanced practice nurses may legally practice”, defining “what they can do for and with patients, what they can delegate, and when collaboration with others is required.” 1 This scope of practices varies not only across APRN roles – CNS, CRNA, NP, CNM – but also across individual states.1 For the FNP specialty, scope of practice entails diagnosis and management of illnesses in the primary care setting for individuals across the lifespan. The three benchmarks of APRN practice and education are advanced practice nurse competencies, professional advanced practice registered nurse competencies, and Master’s and Doctor of Nursing Practice Essentials.2 APN competencies, which distinguish…

Critical care is the specialty I am most interested. I main reason why I wanted to become a nurse, was to become a Certified Registered Nurse Anesthetist (CRNA). I didn’t want to become a CRNA because of the amount of money they made, it wanted to become one, because I had a great encounter with one while I was at the VA hospital having a procedure done. We discuss job satisfaction and how he got to where he is now. Through out the whole procedure he was right by my side explaining what was going.…

This program allowed me to take challenging courses, while also sharpening my laboratory skills. Furthermore, by taking classes with students at New Jersey Medical School, I was able to modify and adapt my study habits to a more rigorous curriculum, similar to that of PA school. During this time, I continued to increase my knowledge as a medical scribe by transitioning from emergency medicine to surgical orthopedics. Here, I was introduced to the role of surgical PAs in an outpatient setting, which exposed me to another side of the PA career.…

Throughout my clinical rotations, I have dedicated myself to giving best shot at all the specialities, and at the same time trying to figure out a speciality that would align flawlessly with my personal and future goals. While this task seemed straightforward and uncomplicated, it was challenging to find a specialty that combined cerebral requirements of internal medicine and procedural skills of surgery, until my Anesthesiology rotation. Major factors that drew me to the specialty were the challenge of managing critically ill patients, the clinical application of pharmacology and physiology and the hands on nature of the speciality. During my third year, I had glimpse of the profession while doing my core rotations. My first month into clinical rotations, I was on-call for my internal medicine rotation.…

One thing that I have learned is the importance of documentation and research to the future role of advance practice nurses. Advanced practice nurses, themselves, play a huge role in the outlook of this field. “We would all do well to remember this underlying philosophy of NP practice, then acquire and utilize skills that honor our nursing roots while providing the highest quality primary care”, Berg, Hicks and Roberts, 2017). Not only does quality of care impact the nurse practitioner (NP) profession, so does ethics and professionalism. Nurse practitioners have an obligation of acting as stewards in the NP role.…

ABSTRACT The use of peripherally inserted central catheter lines for central venous access in patients has increased in recent years despite a lack of evidence regarding safety. A recent survey of invasive catheter practices among patients in the United States found that 37% of terminally ill patients use peripherally inserted central catheters (PICC lines) as part of their treatment protocol. The goal of this paper is to inform the reader about the safety, teaching, and care of peripherally inserted central catheters and lines.…
