Plaintiff, Darryl Juan Irons, SID # 2010927, has filed suit in regard to the reversal Rule 115, which the Office of Administrative Hearings rule should be expunged from his prison file.

On April 19, 2015, Mr. Irons claims that Officer Shropshire advised him that he needed to submit to a urinalysis. Officer Brown provided water and supervised the urinalysis and around 1:35 p.m, Mr. Irons provided his sample to Officer Kouassigan. On April 27, 2015, Officer Melton and Sergeant Onley advised Mr. Irons to pack up his belongings because he was going to lock-up but refused to provide the reason. At the Lieutenant Office, Officer Okankwo presented Mr. Irons with a disciplinary ticket for a Rule 115 (Refusal/failure to provide urine sample for testing; provide diluted urine sample). He was then taken to disciplinary segregation.

Irons filled out a sick call for a copy of his medical records which he received on May 6th. On May 13, 2015, Mr. Irons received a disciplinary hearing before Hearing Office Peter Juknelis which, Sgt. Gaskins also attended. During the hearing, Mr. Irons pled not guilty and alleges that he testified about the side effects of his medication Amitriptyline HCL, which is also known as Elavil. He also contends that the disciplinary ticket incorrectly stated that he had no medical conditions and was prescribed no medications. However, Hearing Officer Juknelis found Mr. Irons guilty of violating rule 115.

As a result of the guilt finding, he contends that he I lost his job as an Inmate Observation Aide, 60 good conduct credit days, 10 days segregation time, 6 months visitation, and Minimum Security Status. On May 15th, 2015, Mr. Irons appealed the disciplinary decision to the Warden Armstead and she affirmed the hearing officer’s decision "Appeal noted and denied this information was taken into consideration at the time of
