Client was oriented 4x, appropriately groomed and attired, and no evidence of perceptual or thought disturbance was present. Client appeared to be at low probability of suicide/DTS as evidenced by her verbal denial. Client appeared to be very unhappy in regards of her concerns. It appeared that client has a strong desire of changing the current situation, however, client struggled with finding a direction to make a change. This was based on the fact that client appeared to be frustrated when she was asked what goals that she would like to accomplish. It appeared that client believes that she lack of the ability to make decisions for herself because she has always been the "follower." Client is given the provisional diagnosis of Adjustment Disorder
