The comprehensive intake assessment (Addiction Severity Index/Teen Addiction Severity Index) is conducted with the client. This includes his/her substance abuse history and family/living situation issues, educational and/or employment history, emotional/behavioral issues and psychiatric history. The counselor also reviews the MHSF-III and asks any appropriate questions based on the client's answers.
