Care plans are seen in every part of nursing from acute care to psychiatry. Each nursing care plan has a nursing diagnosis, goals, outcomes, and interventions that is individualized each client (Perry & Potter, 2014). Client centered care planning is a collaboration between client, nurse, doctors, and other relevant health care professionals that is based on clients being able to identify their own goals and being able to make decisions on how to reach their goals (Perry & Potter, 2014).
With having the client involved with their care, we as nurses turning our focus to are working with individuals to reach their goals, provide coping mechanisms, and develop a plan for recovery (Kneisl & Trigoboff, 2013). Psychiatric care plans are client centered
