    According to the CDC (2016) there are more than 29 million Americans living in the United States diagnosed with diabetes, and 86 million Americans is living with prediabetes. Individuals diagnosed with prediabetes are at increased risk for developing type 2 diabetes if lifestyle modifications are not taken into considerations. The purpose of this EBP change project is aimed at improving patient’s knowledge regarding type 2 diabetes, prevention of type 2 diabetes, proper nutrition and self-management.
