The first step in admission to ATCNE is to speak with the intake coordinator and complete a phone assessment. During the phone assessment, the intake coordinator will get basic information such as age, insurance provider, contact information, and type of service the individual is seeking. According to the CDC (2016) there are more than 29 million Americans living in the United States diagnosed with diabetes, and 86 million Americans is living with prediabetes. Individuals diagnosed with prediabetes are at increased risk for developing type 2 diabetes if lifestyle modifications are not taken into considerations. The purpose of this EBP change project is aimed at improving patient’s knowledge regarding type 2 diabetes, prevention of type 2 diabetes, proper nutrition and self-management. During this EBP change project African Americans participants diagnosed with prediabetes will attend a 6-week culturally sensitive diabetes educational program covering topics involving: type 2 diabetes prevention, diabetes education, eating habits, physical activity, and perception of risk associated complications related to diabetes. The expected outcome of this EBP change project is that participants will exhibit an increase knowledge about type 2 diabetes, improve glycemic control and reduce risk factors to help in the prevention or delay of type 2 diabetes.…
Program Background The theory based program that I am critiquing is called the Grady Nia Project it is a collaboration between Grady Health System and Emory University in Atlanta. The Agency for Healthcare Research and Quality (AHRQ) website (“Culturally Tailored”, 2014), Grady Health System is the fifth largest public health in the United State. Jane McCarthy and Helene Johnson tell about their experiences as an outside teacher teaching on the Navajo Nation. The schools on native nations need to ask what the outstanding educational pedagogies are for the native students and how to implement those practices into the classroom instruction. I can’t believe that we are almost done with the Advanced Leader Course (ALC). I must say, it was a wonderful learning experience. I learned from all the Small Group leaders and my fellow classmate and their respective experiences. As the senior project comes to an end, I am truly excited about the benefit my influence has had on the community.
