Yes, performing a thorough skin assessment is important to prevent infection and development of pressure ulcer. In addition, real-time feedback should also be provided during skin audits to address immediately the concern. Skin assess should be done each shift or more often especially if the patient has a higher risk of having pressure ulcers. In the hospital where I am assigned for clinical, most of the patients there who are admitted are at risk for pressure ulcer or has an existing pressure ulcers.
