According to the textbook, “The term hospice refers to a cluster of comprehensive services for the terminally ill with a medically determined life expectancy of 6 months or less”(Leiyu & Douglas, 273). One thing that separates them from other care providers is that they are less focused on preventative care and more on the comfort of the individual. At the time they had an almshouse but needed a place to to care for the sick poor of the city and were financed through donations instead of taxes. According to Willerson and Kereiakes, these donations could not cover all costs and often turned down patients or required them to pay for at least a part of their treatment.

One way managed care organization have been successful in reducing cost is by utilization monitoring and control. In order to do so, there are three element that include, (1) expert evaluation of what services are need (2) determination of how to provide services inexpensively without comprising quality and (3) review of the process of care (PowerPoint). I find these elements to be essential due to the fact that 10% of patient with chronic/complex conditions account for 70% of health care spending and hospital services cost about 50% of all medical care (PowerPoint).
Overall, I find these elements to be important but also should be controlled, as there have been concerns of our quality of care being affected.

The concept that I will be discussing is personal choices. I find that no matter what age we all like to be able to make our own choices. While earning my STNA I was able to learn about this and how we can integrate it in a nursing home.
