Symptoms: 1. Abrupt high fever, lasts 3-7 days, can relapse a dozen times 2. Many spirochetes in blood 3. Bacteria do antigenic variation. They basically keep relapsing because they change a ton. They are coated by Vmp which is the varying protein. One is expressed at a time. Recombination event moves new Vmp gene infront of the promoter giving relapse. Diagnosis: travel history, blood observation Tx: doxycycline, erythromycin |

B. garinii and B. afzelii (europe and asia) Ixodes ticks = hard body. not passed on to tick babies, they get it from infected people. They have to chew on you for 48hrs to get Lyme disease. Nymphal stage ticks are the primary transmitters to human. Symptoms: 1. Erythema migrans rash Late stage: vary with species 1. Neurologic (bell's palsy) - burgdorfi, garinii 2. Cardiac problems 3. Arthritis (knee unilaterally) - burgdoreri 4. Lymphocytoma - garinii 5. Dermatologic (atrophy) - ACA (acrodermatitis chronicum atrophicans), B. afzelii Dx. ELISA test (Ab test) then Western blot Tx: doxycycline |

Causes Leptospirosis (most common zoonotic infection in the world). Like to live in kidneys, peed out and can hang out in water supplies. Symptoms: 1. Sudden onset of headache 2. high fever (105) 3. Nausea, rash Leptospirosis is self-limiting Dx: Darkfield microscopy MAT test ELISA serology test Tx: doxycycline, you do get protective immunity, but there are many many serovars so not that useful. |
