When do ventricular arise?
Well they arise when the
SA and AV Nodes fail to fire and the intrinsic pacemakers in the ventricles take over.
OR
Ventricular cells get irritable
What cause ventricular rhythms?
-Injury
-Ischemia- irritable
-Drugs
-Trauma
-Hypoxia- cardiac tissue needs 02
-Acid base Disturbances- Acidosis
-Electrolyte imbalances
-Myocarditis- Inflammation
-Cardiomyopathy- Heart tissue stretches and enlarges and disrupts pathways of electrical conduction.
-Increase of sympathy- like in hyperthyroidism.

We will have to reverse all of these to stop ventricular irritability,
How do we treat PVC's?
We usually don't treat unless they are symptomatic :)
- Watch what we do to the client especially activity level.
Well we need to treat the underlying cause and watch for progression meaning look for more irritability and see if the are occurring more in pairs.
What types of drugs do we use to treat PVC"S
Amiodarone- Changes refractory period
Lidocaine- anesthises the troubled area in the ventrical
pronestil- supress the PVC's
What do you do in pt having 10 PVC's a minute what do you do?
(First)!!!Get the pt back to bed and see if PVC's go away, just stop doing what caused the PVC's.
What is an R on T?
Potentially lethal, its the R wave falling on the T Wave of the preceding beat. This will happen in the relative refractory period and can be over come if the stimulus is strong enough. This could cause a run of V-tach.
What is an Idoventricular rhythm?
Less than 40 BPM and arises when the SA and AV node fails to fire. The rate is regular but is about 30 BPM so pt will have profound CO in this case, so pt is in deep trouble!!!
What is Ventricular Tachycardia?
-Is a Burst of 3 or more PVC's in a row
- > 100 BPM
Self limiting would be for ex. if patient had 5 beats at once than it stops
OR
Sustained when they continue across the screen and don't stop know matter what the patient does and this is a medical emergency!!!
What will your patient look like in sustained V tach?
Most likely your patient in sustained monomorphic V- Tach your patient will no real cardiac output THIS IS A MEDICAL EMERGENCY!!! pt Losses Conscious very quickly!!!
How do you Treat V TACH?
-Treat the underlying cause
-Use Lidocaine and Amiodarone to supress.
- But you have to remember to use your ABC's for a pulseless and you will need to call a code blue
What is torsades de Pointes?
It means twisting around points.
- You have polymorphic V-tach
- The rate is 150-250
-Very Lethal, pt will lose concious very rapidly. This comprises cardiac Output!!!
How will you treat torsades de Pointes?
We will need to defibrilate client and give them Magnesium!!
also Epinephrine and vasopressin to suppress this rhythm.
What is Ventricular Fibrillation?
The ventricals are quivering.
-Cant determine rate
You will have to call a code to save the patients life!!!! In V-Fib your patient will dying!!!!
Asytole
Your patient is DEAD!!!!
What does HOT PIE Mean?
Hyperkalemia/hypokalemia, Hypoxia, hypothermia, hypovolemia
Overdose-
Tamponade- Cause heart to stop fluid in pericardium compressed heart so it cant move
Pneumoniathorax- same thing it compresses the heart
Infarction- can stop all electroactivity
Embolism- if big enough you can deprive the electrical system of oxogen.
