Handwashing |
1. Don't let uniform touch your sink 2. Turn on warm water 3. Wet hands and add soap 4. Wash hands with for 20+ seconds (include wrist, nails, in between fingers) 5. Rinse with fingers pointing down 6. Use paper towels to dry hands 7. Use paper towels to turn off faucet 8. Immediately discard paper towels without touching it to other hand |

Beginning Proceedure |
1. Wash hands 2. Assemble needed equipment 3. Go to resident's room, knock and pause 4. Introduce self by name and title 5. Identify resident according to facility policy 6. Ask visitors to leave 7. Provide privacy 8. Explain proceedure clearly to resident 9. Answer resident's questions 10. Allow resident to assist 11. Raise bed to appropriate working height |

Ending Proceedure |
1. Position resident comfortably 2. Lower bed to lowest position 3. Place call light, water and phone in reach 4. Do a general saftey check 5. Open curtain 6. Take care of equipment as facility policy 7. Wash hands 8. Invite visitors back in 9. Record and report abnormalities and completion |

Blood Pressure |
1. Clean ear pieces and diaphragm 2. Position arm resting on surface palm up 3. Wrap cuff around arm with bladder on artery, 1 inch above antecubital space 4. Place earpieces in ears, directed forward 5. Inflate to no bigger than 180 6. Deflate cuff listening for systolic and diastolic 7. Reading must be accurate within 4 mmHg on both 8. Accurate record blood pressure |

Radial or Apical Pulse |
1. Locate pulse 2. Listen for 30 sec x2 or full minute 3. Accurate within + or - BPM 4. Obtain and record an accurate pulse |

Respiratory |
1. Count respiratory rate 2. Accuracy within + or - 2 breaths 3. Obtain and record accurate respiratory rate |

Tympanic Temperature |
1. Place cover on thermometer 2. Ask patient to turn head so ear is in front, put new probe cover on 3. Pull back ear slightly, insert in ear canal towards nose 4. Start thermometer 5. Wait until you hear a beep 6. Obtain and record an accurate Temperature |

Oral or axillary Temperature |
1. Don gloves 2. Ask patient if they have eaten or smoked in last 15 minutes 3. Place sheath on probe 4. Correct placement for obtaining oral or axillary 5. In necessary hold the probe in place for oral reading 6. Leave probe in place until instrument beeps 7. Remove prove sheath from the probe and dispose of it properly 8. Replace the probe 9. Obtain and record an accurate temperature. |
