Males

VSD

True or False: Venous returns are usually normal in TGV babies ?
True

Note: it creates 2 parallel circulations

What keeps TGV babies alive ?
The presence of a PDA and ASD/VSD

Pulmonary artery Aorta (systemic)

How is TGV diagnosed ?
- Within 24 hours of life

- Significant cyanosis at birth

- No improvement wt supplemental oxygen

- Fails "hyperoxia test"

- Echocardiogram ( most definite)

- Cardiac catherization ( occassionally)

What are signs and symptoms of TGV ?
- Severe cyanosis

- Signs of congestive failure

- Excessive sweating (cold and clammy sweat)

- Poor feeding

- Tachypnea

- Lethargic

- Big right to left shunt

What time of shunt is TGV ?
Right to Left shunt
What is the treatment for TGV ?
- Keep infants stable until corrective surgery

- Prostalglandin- to dilate PDA

- Balloon atrial septostomy

- Surgery to cut out a section of septum between the atria

What are the Surgical correction of TGV ?

- “Mustard”Procedure

- The aorta and pulmonary artery are switched

- The coronary arteries are replanted on to the new aorta

- Closure of the VSD and ASD

What is the Prognosis of TGV ?

- With no unusual risk factors, survival post-surgery has improved to 95%, when successful surgery is done as a newborn

- May require additional surgeries if stenosis is noted
