Definition of Actual time critical
At the time of the VSS the pt is in actual physiological distress.
Definition of Emergent Time Critical
At the time of the VSS the pt is not in physiological distress, but does have a Pattern of Injury or Significant Medical Condition which is known to have a high probability of deteriorating to actual physiological distress.
Definition of Potential Time Critical
At the time of the VSS the pt is not physiologcial distressed and there is no significant Pattern of Actual Injury / Illness, but does have a mechanism of Injury / Illness known to have the potential to deteriorate to actual physiological distress.
Pts meet the criteria for major trauma if they have a combination of MOI and what co-morbidities?
Systemic illness limiting normal activity / Systemic ill constant threat to life eg,
- Poorly controlled hypertension
- Morbid Obesity
- Controlled or uncontrolled CCF
- Symptomatic COPD
- Ischaemic Heart Disease
- Chronic renal failure or liver disease.
Adult Trauma Time Critical Guidelines - Actual
Any of:
- Respiratory rate < 12 or > 24
- BP < 90 mmHg systolic
- Pulse > 124
- GCS < 13
- SPO2 < 90%
Adult Trauma Time critical Guidelines - Emergent Pattern of Injury
Any of :
- Penetrating Injuries
Head / Neck / Chest / Abdomen / Pelvis / Axila / Groin

- Blunt Injuries
Significant injury to a single region:
Head / Chest / Abdomen / Axilla / Groin
Injuries involving two or more of the above
body regions

- Specific Injuries:
Limb amputation / limb threatening injuries
Suspectected spinal cord injury
Burns > 20% or involving resp tract
Major compound fracture or open dislocation
Fracture of 2 or more of the following:
Femur / Tibia / Humerus
Fractured Pelvis

Mechanism of injury
- Ejection from vehicle
- Motor cyclist / cyclist impact > 30kph
- Fall from height > 3m
- Struck on head by falling object > 3m
- Explosion
- High speed MCA > 60 kph
- Pedestrian impact
- Prolonged extracation > 30 min

Additional for Paediatrics
- Vehicle rollover
- Fatality in same vehicle

Assess co-morbidities:
- Age > 55
- Pregnancy
- Significant underlying medical condition

Any of:
- Moderate or Severe Resp Distress
- SPOz < 90 % room air / 93% on supplimental O2
- < Adequate Perfusion
- GCS < 13 (unless normal for pt)
Medical Time Critical Guidelines - Emergent
Any of:
Medical Symptoms / Syndromes
- Acute coronary Syndrome
- Acute Stroke
- Severe Sepsis, including suspected
meningoccal disease
- Possible Aortic Aneurysm
- Undiagnosed severe pain
- Need for possible hyperbaric treatment
eg acute decompression illness or
cyanide poisoning
- Hypothermia or Hyperthermia
