decrease renal Na+ reabsorption which reduces H2O reabsorption. Each class works in a different area of the nephron (secreted and concentrated into tubule reaching higher concentrations than in blood). Non-renal AE are low.
Beta-Blockers
antagonism of catecholamines at β-receptors (attenuation of cytotoxic and signalling effects of circulating catecholamines); Decreases cardiac output by decreasing HR and contractility = decrease CO; Initial compensatory increase in PVR; Decrease PR in long run by inhibition of β-receptors in kidney which decreases renin = decrease BP = decrease vasoconstriction and decrease Na+ & H2O retention; Produce resting bradycardia; Reduction in exercise-induced tachycardia; do not use in acute decompensated HF;BB Block SNS Response – important for DM and hypoglycemia!!!
ACE Inhibitors
Reverses vasoconstriction and volume retention that is caused by RAAS activation; reduces afterload which increase SV; increases GFR; increases natriuresis and diuresis; reversal of aldosterone-related volume retention decreases preload; statistically significant mortality benefit for HF
ARBs
Hemodynamic profile similar to ACEIs; lack of bradykinin-related vasodilation; mortality benefit – use when ACI is contraindicated, may be additive when used with ACEI; routine combination of ACEI, ARB and aldosterone antagonist not recommended

initiates compensatory responses: tachycardia, activation of the renin-angiotensin-aldosterone system; give with β–blockers and diuretics; NO formation; K+ channel openers; D1 stimulation
