Name 4 types of seronegative spondyloarthropathies (SpAs)

Why is it called seronegative?
4 types:
1. Ankylosing Spondylitis (AS)
2. Reactive arthritis or Reiter's syndrome
3. Psoriatic arthritis
4. Enteric arthritis or Inflammatory bowel spondylitis/sacroilitis

They are called seronegative because there is no associated specific autoantibody.
What are common features of SpAs? (6)
1. no specific autoantibody
2. Increased frequency of HLA-B27
3. Absence of rheumatoid factor
4. involvement of SI and spinal joints
5. Enthesopathy or Enthesitis - inflammation of enthuses (sites where tendons and ligaments insert into bone)
6. common mucotaneous lesions
What is enthesopathy?
lesion or calcification at ligamentous attachments to bone
Compare and contrast syndesmophytes with osteophytes.

Which one is associated with alkylosing spondylitis?
Syndemophytes - ossification along vertebral ligaments (annulus fibrosis and spinal longituidinal ligaments) and parallel to spine; associated with alkylosing spondylitis

Osteophytes - directed laterally in the horizontal plane; associated with osteoarthris
Acute anterior uveitis is associated with which SpA?
Alkylosing spondylitis (AS)
What is keratoderma blenorrhagica?

Which SpA is associated with this?
Scaling, papulo- to postular lesions on soles of feet.

Associated with Reiter's syndrome
Name an microorganism that is associated with ReA and causes GU infections.

Name 4 enteric bacteria that are associated with enteritis-associated ReA.
Chlamydia trachomatis - associated with ReA and GU infections.

4 Enteric bacteria
1. Salmonella
2. Shigella
3. Yersinia
4. Campylobacteria
Which SpA is associated with sausage digits, esp at DIPs of fingers?
Psoriatic arthritis
Name 2 inflammatory bowel diseases that are associated with enteropathic arthritis (or Inflammatory bowel spodylitis/sacroilitis).
1. Crohn's disease
2. Ulcerative colitis
How are SpAs treated? (i.e. list treatments in least to most aggresive order)
Least to most aggressive (1-5)

1. Rest, physical therapy
2. NSAIDs, muscle relaxants
3. antimetabolites, such as methotrexate
4. corticosteroids
5. other immunosuppresive therapies.
Identify the type of SpA.

Symptoms:

Ankylosing spondylitis
Identify the type of SpA.

Symptoms:
Urethritis/cervicitis

Enthesopathy
Mild sacroilitis
Reiter's syndrome or Reactive arthritis
Identify the type of SpA.

Symptoms:
Sausage digits on DIPs
Asymmetrical joint involvement
Psoriatic arthritis
Identify the type of SpA.

Symptoms:
Associated with ulcerative colitis and Crohn's disease
Enteropathic arthritis
