Compare pt populations w/SLE and drug-induced SLE?
SLE: typically young, black, female
DISLE: typically older, white males with a milder dx than idiopathic SLE
Etiology of SLE?
possible loss of self-tolerance (autoaB production) w/ possilbe hormonal, genetic (HLA?) & environmental triggers (UV light, chemicals/drugs, infections)
Clincial presentation of SLE?
constitutional symptoms (universal), muscoloskeletal, skin, pulmonary, cardiac, CNS, GI, kidney, hematologic-MULTI-ORGAN SYSTEM DISEASE!!
Describe pathophy of SLE?
dvlpmt of excessive autoantibodies & immune complexes; ABs to nuclear, cytoplasmic, & cellular surface components; hyperactive lymphocytes & impairment of immune reg (CD8 supp T-cells release cytokines, Ils, y-interferon, TNF)
Autoantibodies seen in SLE?
ANA, anti-ds DNA, anti-ss DNA, RNA abs, lupus anticoagulant (paradoxical rxn seen)
Describe constitutional symptoms of SLE?
fever, fatigue, wt loss
Describe muscoloskeletal symptoms of SLE?
arthritis, arthralgias, myalgias
Describe skin manifestations of SLE?
butterfly (malar) rash usually post-sun exp, photosensitivity, raynauds phenomenon (cyropercipitate digits turn blue)
describe pulmonary manifestations of SLE?
chest pain/pleurisy, lupus pneumonitis (fever, dyspnea, tachypnea, cough, pleuritis)
describe cardiac manifestations of SLE?
pericarditis, myocarditis, murmurs/EKG changes
describe CNS presentation of SLE?
psychosis, seizures due to inflammation of CNS
DESCRIBE GI SYMPTOMS OF SLE?
N,ab pain, hepatomegaly, splenomegaly, mucosal ulcers
Describe lupus nephritis & glomerulonephritis?
renal involvement=poorer outcome;
ESRD major cause morbi/mortal in SlE
symptoms of lupus nephritis?
proteinuria (3+), casts, hematuria/pyuria in the absence of infections, incr scr, abnl renal biopsy
describe hematological presentation of SLE?
anemia of chronic dx, hemolytic anemia, leukopenia/thrombocytopenia, lupus anticoagulant (antibody against lipid moiety of the prothrombin activator complex causing hyoercoagulability & thrombosis)
