(eg. Poison ivy, nickel jewelry, irritant chemicals, drugs)

What is the difference b/w contact dermatitis and Atopic dermatitis?
Both look same PHYSICALLY (red, itchy, oozing, vesicles/edema)

Atopic associated with asthma and allergic rhinitis
Contact associated with some irritant
What is the treatment for Dermatitis?
Topical Steroids!

(Contact dermatitis: get rid of offending agent)
(atopic: treat/manage asthma and allergic rhinitis)
Acne Vulgaris
Comedones (white or blackheads), pustules (pimples), and nodules

Common in adolescents (duh)

Happens due to chronic inflammation of hair follicles
MOA behind Acne Vulgaris: Comedones

(note: what is it's relationship with adolescents and hormonal changes)
Sebaceous glands produce sebum (oily/waxy liquid). They have androgen receptors thus during teen age years they are highly active.

The sebum is secreted into the hair follicles (located in dermis of skin). Keratin production is also increased. This clogs the hair follicle causing black/white head
MOA behind Acne vulgaris: pimples (pustules)

(note: start from the hair follicle being clogged aka black/white heads)
Since the hair follicle is clogged by keratin and sebum, flow is messed up.

Whenever you have stasis, infection/inflammation may occur.

Propionibacterium acnes infects
When and how does the propionibacterium acnes cause infection?
It breaks down the excess sebum with lipases producing PRO-inflammatory fatty acids.

it infects when hair follicle is clogged (sebum + keratin)
Why use benzoyl peroxide in acne?
Antimicrobial

Kills P.acnes (bacteria)
Why is vitamin A derivatives for acne vulgaris treatment?
They decrease the production of keratin (decrease the clog!)

Vitamin A is good at maintaining specialized epithelium (eg. hiar follicle)

Well-circumscribed, salmon colored plaques with silvery scale;

can have nail pitting

Due to excessive keratinocyte proliferation

parakeratosis = retaining nuclues past the stratum granulosm layer (should go away as you move up)
What is the reason for the silvery scaling in psoriasis?

silvery scaling is due to excess keratin

Auspitz sign:
Bleed when you scrape away the silvery scaling b/c the bv are located in the papillae (DERMIS) which are really close to the surface of skin
What is the treatment for psoriasos?
Steroids

PUVA (psoralen UVA): put psoralen on skin so you can absorb the UVA better. This will destroy the keranocytes
