reduce total number of ischemic episodes, symptomatic and asymptomatic

reduced survival and increased need for PTCA and CABG and increased risk of acute MI
What is initial tx for silent ischemia?
modify major risk factors for ischemic heart disease: HTN, hypercholesterolemia, and smoking
What silent ischemia pt are the best candidates for BB?
post MI and those with high level of sympathetic nervous system activity
Are BB or CCB more effective in silent ischemia?
BB, they both work
Does medical or PCI therapy reduce silent ischemia more?
PCI
How long for symptoms to improve from ischemic heart disease?
2-4 weeks, remain stable until disease progresses
What is goal BP?
130 or less/85 or less with CAD coexisting condition (DM, HF, renal failure)
less than 140/90 with CAD and no coexisting condition
When is BB recommended?
all pts with prior MI in the absence of CI
When is ACEI recommended?
all pts with CAD who have DM and/or LV systolic dysfuntion
